Payment is expected at the time of your office visit. A check, cash, MasterCard or Visa may be used for payment.

INSURANCE:
The Center for Fertility and Gynecology, Inc. participates in Blue Cross and Blue Shield and several other insurance plans.

Please bring your insurance cards to your appointment.

Covered office procedures, surgery and laboratory services will be submitted directly to your insurance company for payment.

Patients should contact their insurance company if there is a questions regarding the extent of coverage for our services.
