New Infertility Patient Information
The initial visit generally consists of a consultation with your and, if desired, your partner, as well as a gynecological examination. If you have not already done so, please verify with our office whether Dr. Tippet participates in your insurance plan. If you are unsure of your benefits for doctors' office visits, you should also contact your insurance company. Please keep in mind that some plans require primary care physician referrals. If you are required to pay for your office visit, the fee for infertility evaluation is $300.00.

Dr. Tippet would like to review your past gynecological or other applicable records; release of records forms are available for download below. Please print and complete the forms, and send or deliver to your previous physician's office.
